ScienceDaily (July 27, 2012) ? Policymakers in the fight against HIV/AIDS may have to wait years, even decades, to know whether strategic choices among possible interventions are effective. How can they make informed choices in an age of limited funding? A reliable, well-calibrated, predictive computer simulation would be a great help.
Policymakers struggling to stop the spread of HIV grapple with "what if" questions on the scale of millions of people and decades of time. They need a way to predict the impact of many potential interventions, alone or in combination. In two papers to be presented at the 2012 International AIDS Society Conference in Washington, D.C., Brandon Marshall, assistant professor of epidemiology at Brown University, will unveil a computer program calibrated to model accurately the spread of HIV in New York City over a decade and to make specific predictions about the future of the epidemic under various intervention scenarios.
"It reflects what's seen in the real world," said Marshall. "What we're trying to do is identify the ideal combination of interventions to reduce HIV most dramatically in injection drug users."
In an analysis that he'll present on July 27, Marshall projects that with no change in New York City's current programs, the infection rate among injection drug users will be 2.1 per 1,000 in 2040. Expanding HIV testing would drop the rate only 12 percent to 1.9 per 1,000; increasing drug treatment would reduce the rate 26 percent to 1.6 per 1,000; providing earlier delivery of antiretroviral therapy and better adherence would drop the rate 45 percent to 1.2 per 1,000; and expanding needle exchange programs would reduce the rate 34 percent to 1.4 per 1,000. Most importantly, doing all four of those things would cut the rate by more than 60 percent, to 0.8 per 1,000.
Virtual reality, real choices
The model is unique in that it creates a virtual reality of 150,000 "agents," a programming term for simulated individuals, who in the case of the model, engage in drug use and sexual activity like real people.
Like characters in an all-too-serious video game, the agents behave in a world governed by biological rules, such as how often the virus can be transmitted through encounters such as unprotected gay sex or needle sharing.
With each run of the model, agents accumulate a detailed life history. For example, in one run, agent 89,425, who is male and has sex with men, could end up injecting drugs. He participates in needle exchanges, but according to the built-in probabilities, in year three he shares needles multiple times with another injection drug user with whom he is also having unprotected sex. In the last of those encounters, agent 89,425 becomes infected with HIV. In year four he starts participating in drug treatment and in year five he gets tested for HIV, starts antiretroviral treatment, and reduces the frequency with which he has unprotected sex. Because he always takes his HIV medications, he never transmits the virus further.
That level of individual detail allows for a detailed examination of transmission networks and how interventions affect them.
"With this model you can really look at the microconnections between people," said Marshall, who began working on the model as a postdoctoral fellow at Columbia University and has continued to develop it since coming to Brown in January. "That's something that we're really excited about."
To calibrate the model, Marshall and his colleagues found the best New York City data they could about how many people use drugs, what percentage of people were gay or lesbian, the probabilities of engaging in unprotected sex and needle sharing, viral transmission, access to treatment, treatment effectiveness, participation in drug treatment, progression from HIV infection to AIDS, and many more behavioral, social and medical factors. They also continuously calibrated it until the model could faithfully reproduce the infection rates among injection drug users that were known to occur in New York between 1992 and 2002.
And they don't just run the simulation once. They run it thousands of times on a supercomputer at Brown to be sure the results they see are reliable.
Future applications
At Brown, Marshall is continuing to work on other aspects of the model, including an analysis of the cost effectiveness of each intervention and their combinations. Cost is, after all, another fact of life that policymakers and public health officials must weigh.
And then there's the frustrating insight that the infection rate, even with four strengthened interventions underway, didn't reduce the projected epidemic by much more than half.
"I actually expected something larger," Marshall said. "That speaks to how hard we have to work to make sure that drug users can access and benefit from proven interventions to reduce the spread of HIV."
Marshall's collaborators on the model include Magdalena Paczkowski, Lars Seemann, Barbara Tempalski, Enrique Pouget, Sandro Galea, and Samuel Friedman.
The National Institutes of Health and the Lifespan/Tufts/Brown Center for AIDS Research provide financial support for the model's continued development.

WASHINGTON (AP) ? Relentless GOP criticism of a 12-day-old remark about business owners has taken a campaign toll on President Barack Obama, forcing him to defend himself and giving Republican Mitt Romney a break from steady attacks.
The development has delighted Republicans, who were eager to shift the campaign focus from Romney's tax returns, overseas assets and Bain Capital record.
Acknowledging that the Republican's criticisms were hitting a mark, Obama's team rolled out two new TV ads this week in which he employed for the first time what many Democrats consider a powerful tool: the president talking directly into the camera and countering GOP claims.
"Those ads taking my words about small business out of context ? they're flat-out wrong," Obama says in the newest ad.
Democrats say the "direct-to-camera" format plays to the president's strength, and they don't think Romney can match it. But like any strategy deployed 15 weeks before Election Day, it might lose some of its impact over time.
In the immediate future, Democrats hope Obama's response will help him move past the flap about business owners. But Romney aides kept up the pressure Wednesday, sponsoring 24 events on the topic while Romney was overseas.
Democratic strategists acknowledged Wednesday that Obama was being hurt, at least a little, by Romney's repeated jabs at comments the president made in Virginia on July 13, which originally drew little notice.
"If you've got a business, you didn't build that," Obama said, in part. "Somebody else made that happen."
Most GOP attacks ignored the broader context of the speech. In it, Obama discussed a favorite theme: the claim that government-assisted infrastructure including roads, research and schools help sustain American society, including private enterprise.
Romney and his allies have used the quote in countless ads, videos, statements and conference calls, painting Obama as contemptuous of hard-working entrepreneurs and business owners.
It's a presidential topic so familiar that few reporters or Obama critics took note of the specific remarks for a few days.
"If you were successful, somebody along the line gave you some help," Obama said in the July 13 speech. He cited teachers and mentors who helped "create this unbelievable American system that we have that allowed you to thrive. Somebody invested in roads and bridges."
"When we succeed, we succeed because of our individual initiative, but also because we do things together," he said.
Late on July 16, the Romney campaign began a drumbeat of attacks quoting only the line, "If you've got a business, you didn't build that."
Democrats responded cautiously at first. Eventually, aides began saying Obama's "that" referred to the infrastructure he cited elsewhere in the Virginia speech.
In an interview with a Florida TV station that aired Friday ? a full week after the original remarks ? Obama said: "What I said was, together we build roads and we build bridges...Anybody who actually watched the tape knows that was what I was referring to."
Obama had planned to address the matter more fully on Friday, aides say. But that day's mass shooting at a Colorado movie theater sidelined both campaigns for several days.
Still, Obama and his advisers realized the attacks were starting to hurt, and they filmed the 30-second response ad in the White House chief of staff's office on Saturday. In the ad, first aired Tuesday, Obama says: "Those ads taking my words about small business out of context ? they're flat-out wrong. Of course Americans build their own businesses."
Obama also began to push back in his stump speech.
"Over the next four months, you are going to hear a lot of stuff," he said at a New Orleans fundraiser Wednesday evening. "Sometimes they will play around with things I say. They'll take out whole sentences. They've got an ad right now where they just spliced it and diced it and make it seem like I don't appreciate the incredible work of small businesspeople."
Romney and the Republican National Committee have since urged voters to review the entire speech. Their criticisms of Obama are fair and in context, they say.
Obama campaign adviser David Axelrod, referring to the first Romney ads about the "you didn't build that" quote, told MSNBC on Wednesday: "I was concerned when I saw the initial ad that it might be impactful. I've concluded that it's not all that impactful. But the ad that we did in response got a very good response in the testing that we did, because it pushes our message forward."
A Democratic strategist with ties to the campaign said Obama's team used focus groups, Web studies and other techniques to gauge the likely impact of Romney's attacks. No Obama campaign ads are aired without undergoing intense study, said the strategist, who would speak only on background in order to discuss private meetings.
Campaigns often use an opponent's remarks selectively, and then weather charges of taking the comments out of context. In January, some Democrats pounced when Romney told an audience, "I like being able to fire people." GOP aides said it was clear that Romney was referring to the right to seek the best providers of services such as insurance.
Romney campaign spokesman Kevin Madden said Wednesday that Obama's July 13 speech "crystalizes the differences" between the two candidates on how large and active the federal government should be. The lesson of the Republicans' 2010 election victories, he said, is that "Washington has to stop growing the size of government."
Obama aides said it's largely coincidental that his new 30-second ad looks much like a 60-second ad that was recorded some time ago but first aired Monday. Both were filmed in the White House chief of staff's office, and consist of the president speaking directly to viewers.
The 60-second ad, titled "The Choice," contains none of the defensiveness of the shorter one. It pursues the Obama campaign's oft-stated goal of framing the election as a choice between two visions of America more than as referendum on Obama's first term, which has been dogged by high unemployment.
"Over the next four months, you have a choice to make," Obama says in the ad. "Gov. Romney's plan would cut taxes for the folks at the very top. Roll back regulations on big banks. And he says that if we do that, our economy will grow and everyone will benefit." That approach has failed in the past, Obama says.
Romney's campaign shows no sign of letting up on the "you didn't build that" attacks. The president's comments made Florida business owner Lou Ramos "almost throw up," the campaign said Wednesday in announcing an event in Tampa featuring Ramos.
The Democratic National Committee responded with conference calls in which elected officials and small business owners denounced the "out-of-context attacks on the president."

A woman who said she was sexually assaulted while on a two-month row around Lake Michigan to raise cancer awareness says she remains frightened but plans to resume her trip Wednesday.
Jenn Gibbons is trying to become the first person to solo-row the entire 1,500-mile perimeter of Lake Michigan in an effort to show the role exercise plays in the fight against breast cancer. She set out on her two-month mission from Chicago on June 15, NBCChicago.com reported.
About five weeks into her journey, she said, she awoke in the early morning hours on Sunday to the sound of someone boarding the boat, which she had docked for the night near a lighthouse on a stretch of a remote shoreline of Lake Michigan. Gibbons told police she rushed to lock the cabin door, but the stranger forced his way inside and sexually assaulted her.

?I was out in the middle of nowhere,? Gibbons told the Chicago Sun-Times. ?It was terrifying. And he told me he knew where to find me.?
Police believe Gibbons? attacker traveled a significant distance to find her, they said in a statement, perhaps following her on the Internet as she documented her mission through her Facebook page and personal blog.
Gibbons said she punched the man in the chest and ran. She tried to lock herself in a wooden outhouse, but her attacker forced his way inside. She said she fought him off until he fled in a yellow Jeep Wrangler.
Gibbons said she ran to the boat, locked herself in the cabin and called 911, police said.

Gibbons said she hoped her mission circumnavigating Lake Michigan would raise $150,000 for her charity. She has already raised $80,000, the Chicago Sun-Times reported.

They are well-written, insightful and generally thought-provoking, which is why I keep reading them. There is just one problem in our virtual relationship: I am a woman (like the majority of personal finance blogs? readers). If one were to take seriously the comments of bloggers and male visitors alike on some of these sites ? this one excluded ? womanhood is a mode of existence irreconcilable with good money management skills. If I had a nickel for every time I?ve read a comment about the terrible financial choices made by a man at the instigation of his partner ? I would be at least a few hundred dollars closer to retirement.
And my only response to all those laments of financial woe is, simply put: Shut it!
Here?s why.
Financially responsible, high net worth women exist. They are not a figment of feminist propaganda or neutered personal finance blogs. They walk the earth among you. If you, as a financially responsible guy, end up marrying, shacking up, or making babies with anyone who does not fit that description ? and wish you had ? you have no one to blame but yourself. If you then let that person influence your financial decisions, or worse yet, abdicate your personal responsibility and place the decisions entirely in her hands, you have no one to blame for the result but yourself. Sounds harsh? Let?s explore.
First, I don?t know of any studies that have ever conclusively proven that women, as a group and solely on the basis of gender, are more financially irresponsible than men. If such a study exists, I hope that Joe will bring it to my attention. Absent proof of this beyond the purely anecdotal, I will wager that, generally speaking, men are not facing insurmountable statistical odds in the quest for a financially responsible partner. The question is: what premium do men put on a woman?s fiscal responsibility? Where, on the list of necessary attributes in a life partner and/or co-parent, do money management skills land? Near the top? Near the bottom? Nowhere? What other desired attributes are they willing to forgo in the interests of securing their prospective families? financial security? Is it more important that your girlfriend or wife look like Kate Upton, or that she can balance a budget? Is it more important that she allows you to feel like a swell guy, or that she can bring in a great pay-cheque (bigger, perhaps, than yours)? These are not either/or questions necessarily; plenty of women with enviable assets (financial and physical) look ? if I may be prurient for a moment ? great in a bikini, and they are not all harpies intent on emasculating their partners. But if in love, as in life, there have to be compromises. Every man must make his choice ? and live with it ? or her, in this case. And then man up, and not go whining about it later in the comment sections of various sympathetic personal finance blogs.
Second, apart from those who might legitimately deserve the label of ?sociopath?, most people do not and cannot disguise their real selves for long. You might not be able to tell immediately whether the wonderful girl you just started dating has a ton of disposable income, or 5-figure credit card debt instead; after all, it?s not something generally disclosed in the preliminary movements of the dating game. But, barring whirlwind romances or arranged marriages, by the time the game has progressed far enough to require a decision as to your joint long-term future, you should have a pretty good idea about not only the contents of her bank accounts, but also her entire fiscal philosophy ? and if either or both of those pieces of information make you feel queasy, then run for the hills ? or not. And if you don?t, then man up and don?t whine about it later on the?personal finance blogs of yours and others. Wait, did I already say that?
If, against all better judgment, you married or otherwise yoked yourself to a woman you know to be incapable of living within her means or completely divorced from the reality of a hard-earned dollar, prepare yourself. Be ready to spend the rest of your life standing your ground on decisions that are likely to be very unpopular with your partner. Hey, she must have qualities that make up for that inconvenience, or else you wouldn?t have married her, right? But if it happens that, for any reason ? be it exhaustion or apathy ? you acquiesce in the choices she makes, or leave the whole decision-making process in her hands, accept that it is as much your responsibility as if you had made that decision alone. Short of actual coercion, no person can force another into a bad decision ? be it an unwise purchase, an adulterous liaison, or a bank robbery. Anyone who tells you otherwise is evidently unfamiliar with the concept of personal responsibility. A moribund idea in our society for sure, but one we should not be so ready to pull off life support just yet.
In summary, dear male blog writers and their fanboys: please don?t assume that, my bikini-worthiness notwithstanding, my gender dooms me to a life spent enriching the pockets of my credit card provider. In return, I promise not to make assumptions about your love life, or the acute bitterness engendered by the lack thereof. PHILADELPHIA The percentage of HIV patients taking antiretroviral drugs who experienced the full benefit of the drugs jumped from 45 percent of 72 percent during the past decade, a figure that is lower than previous estimates. The findings, considered important for HIV prevention efforts, since patients whose virus is in tight control are less likely to transmit the infection to others, are published this week in JAMA by a team of researchers led by the Perelman School of Medicine at the University of Pennsylvania and Johns Hopkins University School of Medicine. The issue's publication coincides with AIDS 2012, the annual international AIDS conference, being held in the United States for the first in over 20 years this week in Washington, D.C.
The researchers analyzed 32,483 HIV-infected patients cared for in 12 clinics across the United States between 2001 and 2010. patients in the U.S. During that time, the percentage of patients taking antiretroviral drugs who exhibited sustained viral suppression having no detectable HIV virus in the blood every time the virus is measured -- increased from 45 percent to 72 percent. The authors point to new drugs and fixed dose combination tablets as factors in improving the efficacy and safety of antiretroviral drug regimens, as well as decreased side effects that make patients more likely to adhere to their treatment. Better access to care during this time period may also be a contributor to the improved viral suppression rates observed in the study.
Despite this increase, the number of patients with tightly controlled HIV infection was significantly less than the 77 percent to 87 percent figures reported in prior studies, which were based on one-time only measures of HIV virus in the blood, rather than considering every time the virus was measured. According to the study's lead investigator, Baligh Yehia, MD, MSHP, MPP, a fellow in the division of Infectious Diseases at Penn Medicine and HIV specialist, "if the HIV virus is not fully suppressed, individuals are at risk of transmitting HIV to others in the community." Yehia says that while significant advances in HIV therapy have occurred over the past decade, consistent adherence to medicines and outpatient care remains a major hurdle for many people living with HIV. The findings come on the heels of the Food and Drug Administration's approval this month of the first drug to prevent HIV infection. Similar to people living with HIV, healthy individuals without HIV infection who chose to take this preventative medication need to have strict drug adherence and frequent HIV testing in order to achieve benefit.
Overall, sustained viral suppression was lower for black patients and injection drug users, while older patients and those with private insurance were the most likely to have sustained viral suppression. "Our analysis showed that HIV care providers need to closely monitor younger patients, African-Americans, injection drug users, and individuals without health insurance to ensure that they have access to HIV drugs and adhere to therapy, since those patients appeared to be least likely to have their infection in tight control," said the study's senior author Kelly Gebo, MD, MPH, an associate professor of medicine at the Johns Hopkins University.
"There is no denying the progress we have made over the past decade," says Yehia, "but, more resources and new technology are needed to ensure that more patients have access to HIV therapy and obtain the full benefits of these drugs."
